Required under Rowan’s Law (Concussion Safety), 2018 in Ontario.
Ensures all participants review concussion education materials every year before playing.
Applies to players, coaches, officials, and team staff.
Before participating in soccer, you must:
Review:
BMSA Concussion Code of Conduct
Age-appropriate concussion awareness resources (from Ontario government)
Sign the acknowledgment form
If under 18 → a parent/guardian must also sign
Wear proper equipment correctly
Follow the rules of soccer
Practice fair play and respect others
Understand:
A concussion is a brain injury with short- and long-term effects
It can happen from any blow to the head or body
Loss of consciousness is not required
You agree to:
Report your own suspected concussion
Report if you think someone else has a concussion
Share relevant injury information with:
Coaches/team staff
Schools or other sports organizations
Report concussions that happen outside of soccer
Complete and submit injury reports
Encourage open discussion about concussion symptoms
Support athletes in speaking up about injuries
Maintain communication with players and parents
You must follow these strictly:
If a concussion is suspected:
Immediate removal from play
No return until medically assessed
Must be cleared by a:
Medical doctor or nurse practitioner
Respect decisions of:
Coaches
Healthcare professionals
Follow proper return-to-play protocol
You cannot participate without signing this form.

Honest reporting
Proper medical care
Everyone shares responsibility for preventing and managing concussions.
